This is a colorful 1936 series Treasury Department IRS Narcotics form. The document is dated and filed 1947 at Cases Drug Store, Logan Ohio. This decorative record confirms that the narcotic was Codeine Sulphate 1/2 gr. and was purchased by the Rexall Drug Company, Pittsburgh, PA. The form measures 11" high x 9" wide and is in fine condition except for a 1/2" tear in the upper (R) hand corner (barely seen when framed). Although once common, these decorative documents are getting more difficult to find. |
